Abnormal origin of the posterior inferior cerebellar artery

Case report

✓ Posterior inferior cerebellar arteries originating from the vertebral arteries between the axis and the atlas were demonstrated angiographically in a 12-year-old boy. The patient presented with tetraparesis of sudden onset after a gymnastic exercise. There was no recognizable trauma.

The posterior inferior cerebellar arteries (PICA's) on both sides are seen originating from the third segment of the vertebral artery, entering the vertebral canal between C-1 and C-2, and running upward lateral to the spinal cord (arrowheads). A second small vessel originates from each vertebral artery just below the vertebrobasilar junction (arrows) and joins the abnormal PICA of the same side.
