The article by Wenger and Markwalder in this issue of Journal of Neurosurgery: Spine is intriguing and compelling; however, we have several major concerns about this study, including its lack of a control group and the significant unintended bias on the part of the treating physicians and the patients themselves. The desire to achieve a good result, the placebo effect, and intellectual bias can create an aura of undue and inappropriate optimism, as has been demonstrated repeatedly in the medical literature.
